It's worth noticing that from table 1 to 2 all that has happened is we have taken the note on the low E and put it on the high one. Also worth noticing that as you go up each string the chord is spelled as well. I hope the fingerings are self explanatory. I leave it to the ambitious to extend these forms past the the twelfth fret.
